I think if it's just a place to sign you should write "signature of witness" or

But if there's a long section describing what the witnesses saw and heard the section should indeed be titled "Affidavit of Witnesses." (See web reference.)

I happen to have such a witnessed document (2 witnesses, each signed in the presence of the other + a third person, and it says העדים on one page with signatures and their information, and then on another page it says: הצהרת העדים and again all their details. So I would tend to think what you have is simply an alteration with "added personal style" of the original author. Possible?
