To Asker The certificate must surely mention the name of the student before the phrase you are struggling with. You can either use the student's name (for "il medesimo") or you can say something like "the student mentioned above".

You can't say "the same" - it's 19th-century legalese, out of place on a degree certificate. The same goes for "selfsame".
