In order to improve visualization laterally, the medial head of the gastrocnemius was divided one fingerbreadth beneath the tendon insertion. Stay sutures were placed into the proximal and distal tendon to allow easy apposition on closure. The PCL avulsion fragment was identified. This was found to be loose and ununited.
The fragment was mobilized and cleared of soft tissue. The defect site was again cleared of soft tissue to allow reduction of the fragment. An acceptable reduction was achieved. This was held with K-wire and then a cannulated drill placed over the top of this.
