medical interview OR history taking

History taking and patient examination are based on principles of elucidating information relative to the patient's health status
http://www.chiropractic.org/Guidelines/ChapterSeven.pdf

Opening the Interview
It is important to begin each medical interview with a patient-centered approach
A minimal interview consists of the chief complaint and primary history of present illness. The other elements may be selectively omitted as circumstances dictate. For example, a complete review of systems is often not necessary or desirable in the context of a focused evaluation

Peer comment(s):

agree Marilyn Amouyal : History is sufficient. In the report it is followed by physical examination, lab and imaging data, hospital course and disposition.

Questionaire

I usually translate it by 'questionaire' or 'questioning'.
I understand it's the first dialogue you have with your physician to establish the reason for the visit and your medical history.
This usually takes the form of a questionaire.

My opinion.
Peer comment(s):

agree Maria Constant (X) : Perhaps we could add oral to be even more precise

neutral awilliams : QuestioNNaire - if this ends up in the glossary will you make sure the sp. is correct - cheers

agree Randa El Nomeir : agree with Maria too. add oral

neutral Angela Dickson (X) : in my experience a questionnaire is standardised and written, and not quite the right term here (though your description is correct)

Anamnesis

It is the proper medical term to convey the exact meaning at hand. :-)
Peer comment(s):

agree Andreea Vintila : definitely (though history taking is not bad either)
