This is not an abnormality of the tibia, but an accessory os naviculare (one of the tarsal bones).

agree Anne Schulz : I think the report explicitly says "larger os tibiale externum" because a small one usually does not produce any symptoms.//"...a relatively large..." then -- well, relative to what? "...a fairly large..."?

congenital tibial deformity / of the tibia

"Anlagevariation" could be something like "angeborene Fehlbildung", i.e. "congenital deformity"...
