Your view is correct! I got the feeling that this sentence should be understood as the pay for preventive service is not included or accumulated when we consider the maximum pay of a covered year.

Actually, the maximum here refer to the maximum pay per medical service within a covered year in insurance as I understand in the context.

Or maybe I should use a neutral expression. Count against may mean "affect" or "influence".

I got the feeling that this sentence should be understood as the pay for preventive service is not included or accumulated when we consider the maximum pay of a covered year.
