jurat loose leaf certificate

I think this is merely referring to a jurat certificate in a loose leaf format, meaning the blank form which you can keep in a loose leaf binder.

I am not a legal expert, but here's one defintion that seems to match much of what I saw in a Google search, all of which pointed to a certificate signed by a notary.

A jurat is a certification on a legal document which provides information about when and where the document was signed, and who witnessed the signing. Lawyers and notaries can both generate jurats which can be used to validate legal documents. A jurat is usually necessary before a document can be legally recognized.

HERE IS AN EXAMPLE FROM THE STATE OF COLORAO on Jurat Statement.

1. No Notarial Certificate (Jurat)

Remember: Colorado Notary Law requires that a jurat statement appear on the document whenever a notarization is performed. When a person goes before a notary public to sign a document, the notary must perform one of two notarial acts:

Administer an oath or affirmation to the person signing the document

Take an acknowledgement from that person regarding the execution of the document in the notary’s presence.
The jurat statement regarding the notarial act he/she performed during that notarization must include:

The type of notarial act performed by the notary

The county and state in which this notarization took place

The date of the notarization

The notary must also include his/her official notary signature, seal, and commission expiration date.

I think that loose in this case refers to a blank certificate, or a blank standard form of a certificate.
