I managed to find the following information online:
• De ziektekostenverzekering is afgestemd op het gezin, niet op het individu. De tegemoetkomingen in de ziektekosten worden immers niet alleen verleend aan zij die persoonlijke aanspraken kunnen laten gelden, maar ook aan de personen ten hunnen laste.
• Degene die kampt met een ziekte en aanspraak kan maken/recht heeft op een vergoeding noemt men gerechtigden (Art. 2, k Ziektewet). De gerechtigde en het gezin samen noemt men de rechthebbenden (Art. 2, j Ziektewet).
• De personen ten laste van al die gerechtigden hebben eveneens recht op ziektekostentegemoetkoming en zijn daarom dus rechthebbenden.

You've already established that the rechthebbende in this case is the patient, assuming the form has been correctly completed, so your question is about gerechtigde.

Here's a similar form which possibly implies that it means another family member:

so maybe in some cases the claimant is claiming on behalf of their spouse or children, but in this instance the patient and the claimant are the same person.
