at the bottom of the birth certificate followed by signatures. I have several possible translations but none seem right. Have checked past KudoZ questions, but answers don't seem to fit.

Based on several years of translating such stuff. Van den End recommends omitting 'waarvan akte,' however, most, if not all, Dutch civil-law notaries would strenuously object. This is still given in various curricula for NL>EN Dutch civil-law translation workshops designed for civil-law notaries.
