číslo jednací změny

I think that "čj. Zm." may perhaps be the reference number of the change of the trade license? "ev.č. 123456-1234567" should correspond to "identification number".

Thank you for your help, "cislo jednaci zmeny" explains why there is a "j" in "cj"...I could not seem to be able to decipher it..
