Japanese term

身分証明書

Japanese to English Law/Patents Certificates, Diplomas, Licenses, CVs
I could not find the standard translation for the official document called 身分証明書. It is not used as an ID document despite the name... It is issued by the government and proves that you are not incompetent or bankrupt.

status certificate by the municipal government (of one's permanent domicile)

Status certificate by the municipal government

Another idea is simply to say "certificate of non-bankruptcy" or "certificate of non-incompetence", etc., depending on the purpose for which the certificate is used, because 'status' can be anything, such as marital status and visa status.
Note from asker:

personal identification

This is the Japanese word for "identification" and refers to all official forms of picture id issued by the municipal or federal government, including a valid drivers license, passport, military ID, green card, gaijin card, etc.

Certificate of Personal Status

Maybe "Certificate of Personal Status" would work
Note from asker:
I think it would work! I am still thinking of what would be best...sorry I couldn't award Kudoz points to more than one person...
