I think this is a specific display which appears once the previously entered data have been processed correctly and which is used to enable/activate the Sourcing Gare (???) service.
I would prefer 'enable' because this is the commonly used term in conjunction with services.
Another option would be 'activation screen'.

I think that "mask" is a highly technical word in English with a slightly different meaning from "maschera" as it is commonly used in Italian. Generally the "maschera" is almost always in a data input box.

Could be a list of options/newsletters etc. that the client has to click on (I want this, I don't want that) once they've registered.

I often see maschera translated as "form", although in this case it sounds more like a column of options the user can select. In that event I'd call it a dialog. If you have a screenshot, Pauley, you'll be in a better position to decide what to call it. Irene's suggestions of "screen" could work well too, although I wouldn't use enablement.
