Source text - Turkish Although the majority of diabetes-related complications are cardiovascular in nature, diabetes is among the leading causes of kidney failure. Approximately, one-third of patients with
diabetes will eventually have progressive deterioration of renal function (Remuzzi, et al 2002), and 10–20% of patients with diabetes will die of kidney failure (WHO 2006b). The first clinical sign of renal dysfunction in patients with diabetes is generally micro-albuminuria (a possible sign of endothelial dysfunction that is not necessarily confined to the kidney) (Ritz 2003), which develops in 2–5% of patients per year (Adler, et al 2003, Gall, et al 1997). In type 2 diabetes, microalbuminuria progresses to overt albuminuria/proteinuria in 20–40% of patients (Nelson, et al 1991). In 10–50% of patients with albuminuria/proteinuria, chronic kidney disease develops that ultimately requires dialysis or transplantation; i.e. end-stage renal disease (Brenner, et al 2001, Lewis, et al 2001, Nelson, et al 1988). Furthermore, 40–
50% of patients with type 2 diabetes who have micro- albuminuria eventually die of cardiovascular disease (Eurich, et al 2004, Parving, et al 2004a); this rate is three-times higher than that for death from cardiac causes in patients with diabetes and no evidence of renal disease (Adler, et al 2003)

Diyabetli hastaların yaklaşık üçte birinde en sonunda böbrek fonksiyonunda progresif kötüleşme görülmekte ve diyabetli hastaların %10 ila %20’si böbrek yetmezliğinden ölmektedir (WHO 2006b). Diyabetli hastalarda böbrek yetmezliğinin ilk klinik belirtisi genellikle mikroalbüminüri (mutlaka böbrekle ilişkili olması gerekmeyen bir olası endoteliyal disfonksiyon belirtisi) olup, yılda hastaların %2 ila %5’inde görülmektedir (Adler ve ark. 2003, Gall ve ark. 1997).
